💥 Fracture (골절)

🩹 Cast Care (석고 간호)

• Drying 중에는 손바닥으로 다루기 (손가락 X)
• 간지럽다고 이물 삽입 금지
• 환측 상승(elevate) – edema 예방
• 냄새/열감/통증/청색증 → 감염 또는 혈류장애 의심
• Plaster cast는 완전 건조까지 24–48시간 소요

💡 Cast cut 시 “tingling / burning” = 정상, 하지만 “numbness / pallor” = 즉시 보고!

⚙️ Traction (견인)

Skin traction (Buck’s)	- Short-term (48–72h) - Hip fracture pre-op stabilization - Weight 2.5–5kg - Heels off bed!
Skeletal traction	- Pins/wires directly to bone - Pin site infection 주의 (serous drainage OK, purulent X) - Never remove weights

⚠️ 견인줄은 항상 자유롭게 매달려 있어야 함 (not touching floor or bed)

🦿 Amputation (절단 간호)

• 초기 24–48h: 절단 부위 상승 금지 (contracture 방지)
• 48h 이후: prone position 30분씩 하루 3–4회
• Stump dressing: figure-8 bandage → swelling 감소
• Phantom pain → Gabapentin / mirror therapy

🚨 급격한 통증·출혈·냄새 = 감염 의심 → 즉시 보고!

🏃 Joint Replacement (인공관절 수술)

Hip replacement (THR)	- Avoid hip flex > 90° - No crossing legs - Use abduction pillow - Avoid internal rotation - Chair: high seat preferred

• Risk: postmenopause, steroid use, immobility

• 공복 아침에 복용 + 물 240mL
• 복용 후 30분간 눕지 않기 (식도염 위험)
